Medical Forms


A medical Form is required as part of Boy Scouts of America (BSA) insurance requirements.  The Form available for print and download.

All Scouts, Troop Leaders and Anyone Camping with the Troop is required to fill out the form.

IMPORTANT NOTES:
1. You must provide the original and one copy to Trip.  You should make a copy for your records.
2. You must make a copy of the Front and Back of you Insurance Card for each Form submitted.
3. Parts A, B and C are required for Summer Camp.
4. A Doctors exam and signature is required on part B.
5. You must provide a signed note is there are medication changes made that are not reflected on the form.

Tips for Scouts going to Summer Camp


Uniforms: Scouts are required to have a Class "A" Uniform at Summer Camp.
NOTE for new Scouts: A Uniform Shirt and Hat are the minimum requirements for Camp. Shirts are required for Dinner.

Personal Gear: Scouts should pack personal gear in a foot locker or trunk that can be locked. A combination lock is recommended since keys can be lost. Writing the combination down and giving it to the leaders is a good backup if the numbers are forgotten. Here is a sample foot locker:

 


ICE: we could use some gallon bottles filled with water and frozen. This will help keep our water and drinks cold longer into the week.

Money: Scouts should have money for any activities as required.

ALSO, Scouts need some spending money for the Camp Store. We recommend you put the money in a zip lock bag, and an envelope with the Scouts name on it. The Adult Leaders will have a lock box to hold the money. We will make every effort to ration the money out so that Scouts do not run out in the first day or two. If Scouts keep the money, then we are not responsible for claims or lost or stolen money.
Getting to Camp: Parents are responsible to get there son to camp. Please plan to arrive at camp on time as Troop must have all Scouts on hand to check in.

ALSO: Plan to eat Lunch before arriving at Camp. Dinner will be our first meal.
